Birth Certificate
Valid Passport
An Affidavit signed by a solicitor declaration to confirm who you and your present status(Unmarried/divorsed/widow)
If either party was previously married, Original divorce decree and if either party was widowed, Original death certificate your late spouse and previous marriage certificate.
If adopted, an adoption certificate
